This page provides quick access to some of the forms families most commonly need at North Mead.

If you are unsure which form to use, or need help completing one, please contact the school office.

Nursery Waiting List

If you would like to register your child for a place in North Mead Nursery, please complete the Nursery Waiting List Form.

Once completed, please return the form to the school office with your child’s birth certificate, as requested by the school.

Apply for Free School Meals

Families who may be eligible for benefits-related free school meals are encouraged to apply.

Even if your child already receives a free meal because they are in Reception, Year 1 or Year 2, an eligible application can also help the school receive additional funding to support pupils.

Applications are made through Leicester City Council. The current North Mead page already links directly to the council’s free-school-meals application service.

Exceptional Pupil Leave

Parents and carers who wish to request leave during term time should complete an Application for Exceptional Pupil Leave.

Completing the form does not automatically mean that the absence will be authorised. Requests will be considered in line with the school’s attendance procedures and current regulations.
